All details collected from this form will be used for the digital contract that you and EMBLAZE Photography will sign.
TO BOOK: We require 50% (for $1500 or less order) or 35%-40% (for larger total) down payment / a signed agreement
All prices are subject to HST @ 13%.
Please note that your booking is not guaranteed until the contract is signed and the down payment is received.
